We want your experience here to be a pleasant one and would like to provide you with some information about SmileOra Dental Plan, a program solely administered by SmileOra, LLC. It is mainly focused on patients who do not have dental insurance. Membership fee of $10/year is included in the plan.10-25% Discounts (Depending on Treatment Plan) will be given on dental treatment other than cleaning & x-rays. The applicable rules & agreement will be automatically applicable every year when you accept and pay for the new plan each year

v  SmileOra Dental Plan is not insurance. It is a discounted fee-for-service plan for self-pay patients. We will charge SmileOra Dental Plan members according to the plan you choose. SmileOra dental does not reimburse you for any portion of the dental services you receive. Payment is made at the time of the service.

v  Presently, the following plans are available, which are subject to change with or without advance notice. Plan fees are paid in advance and in full. Plan fee is non-refundable. The Plan fee applies from January 1^st and ends on December 31^st of current year

v  Renewal of Plan every year will get additional discount as follows. Renewal of 3 or more family members will get additional $5 off the plan. Renewal of family membership has to be done on the same day.

q  Plan A ($110): Includes 1 Cleaning, Fluoride treatment & 1 Exam/year.

q  Plan B ($150): Includes 1 Cleaning, 2-4 BW X-ray, Fluoride tx & 1 Exam/year.

q  Plan C ($250): Includes 1 Cleaning, Full set of X-rays and 1 Exam per year.

q  Plan D ($250): Includes 2 Cleaning, 2-4 BW X-ray, Fluoride tx & 2 Exams/year

q  Plan E ($350): Includes 2 Cleaning, Full set of X-rays, Fluoride tx & 2 Exams/year.

q  Plan F ($350): Includes 3 Cleaning, 2-4 BW X-ray, Fluoride tx and 3 Exams/ year

q  Plan G ($450): Includes 3 Cleaning, Full set of X-rays, Fluoride tx & 3 Exams/year

q  Plan H ($450): Includes 4 Cleaning, 2-4 BW X-ray, Fluoride tx & 4 Exams/year.

q  Plan I ($500): Includes 4 Cleaning, Full set of X-rays, Fluoride tx & 4 Exams/year.

q  Plan J ($150): Includes Full set of X-rays & exam per year.

q  Plan K ($90): Ages 2-12- Includes 1 Cleaning, Fluoride tx & 1 Exam/year.

q  Plan L ($120): Ages 2-12- Includes 1 Cleaning, Fluoride tx, 2 BW & 1 Exam/year.

q  Plan M ($160): Ages 2-12- Includes 2 Cleaning, No X-ray, 2 Fluoride & 2 Exams/year.

q  Plan N ($200): Ages 2-12- Includes 2 Cleaning, 2-4 BW X-ray, 2 Fluoride & 2 Exams/year.  

q  Plan O ($25/Sealant): All Ages, Any Teeth-Minimum 4 Sealants required per visit

v  Discounts will be given only if member pays in full for the service rendered at the time of visit.

v  Unused services will not be carried over to the following or subsequent years, Nor any refund will be given

v  The senior citizen discount or insurance discounts does not apply to the above plans.

v  There are no claim forms, no deductibles, no pre-existing conditions, limitations, no calendar-year maximum & no waiting period.

v  Plan Fees may be updated annually and is subjected to change with or without advance notice.

Reducing our fees simply gives us an opportunity to introduce new people to our practice, and offer them a way to save money on their dental care.
